CASE MANAGER CONSULTANT JOB FOR NEW JERSEY BS1645 (Ref: BS1645)

Location: near Princeton
Salary:$ 80 to $ 90 K (permanent) + Full benefits
Work Experience
Associates Degree in healthcare related field. Bachelor's degree preferred; RN is preferred; LPN may be considered; minimum of 5 years of utilization review/case management experience or social work experience; minimum of 2 years experience in discharge planning from a hospital is preferred; must maintain current professional licensure in nursing or professional field of certification; appropriate certification in case management is preferred. Must have strong presentation skills, ability to educate physician and clinical staff, and critical thinking skills required.

Job Description

Healthcare consulting firm in New Jersey is seeking a Clinical Case Manager Consultant. The ideal candidate will be required to provide consulting expertise through comprehensive assessment, planning, implementation and overall evaluation of procedures and services provided by healthcare providers. This role requires knowledge and skills in Case Management that one can translate to create and manage services that will enhance the quality of patient management. The position will provide hospital case management/utilization review and discharge planning, and will coordinate hospital activities concerned with case management and discharge planning. Will also conduct concurrent medical record review using specific indicators and criteria as approved by regulatory agencies inclusive of CMS, state agencies and JCAHO. The incumbent can work remotely with roughly 60% travel required to mostly local hospitals. Qualifications: Associates Degree in healthcare related field.
